Procedure for getting the MEMBERSHIP of AMDwA

Step 1. Download & Fill out the form given below.

You must fill out the downloaded 3-page form, and sign on each page.

Requirements :

Completely filled and signed form.
ARN Holder photograph to be pasted on Page No.1
Self Attested copy of ARN Card

Step 2. Make Payment

You must deposit a fee of Rs 501/- in the AMDwA bank account or pay Online Here. mention your ARN number in remarks while making online payments.
